Most of those I know who are near the limit of 100, have more than just friends on their xbl-list; they have an entire set of folks added for 1 particular game or another, such as 70+ for MW2 or 60+ for H3. One even has two gamer tags, one for her competitive H3 clan play and the other for all other games.
When I was spending a lot of time in GoW2, I added a few just to have decent strangers for horde matches. Their matchmaking was a bit of a hassle, so temp friends made finding matches easier. After I or the others finished playing GoW2, the temp-friends were removed.
I agree with RobertR1, they need grouping. It was barely manageable for me to navigate through 40 friends to see who was doing what in a timely manner, I can't imagine going through 100 or 200, let alone 1000.
