Could very well be no actual "folding" (which would require lots of geometry) but "just" a clever shader effect. Regardless, the result is quite impressive.

Do you know the first thing that makes that look absolutely nothing like sweat coming down a dirty face?
And if they're trying to suggest they have the skin modelling down that well it's something that should be perfectly simple in comparison...
He has a strap from an eye-patch running over his cheek and yet the "sweat" line runs perfectly through it. If that was real sweat then it would run down to the strap, puddle on it and likely run back along the strap to the lowest point, pooling there until there was sufficient fluid for it to break surface tension and run over the strap. Now if they have honestly spent that much time modelling skin motion surely some simple fluid physics are small effort in comparison?
