For AAA games they don't, even if the character has a nude scene. But for some reason, they modeled really all parts of Ellen Page for Beyond.

Bad mistake.

Usually it's Barbie/Ken level, although the zbrush sculptors tend to do a complete job on full bodies.

Then there's the case where the client actually wants (usually partial) nudity... We did have naked women in AC4 and WatchDogs, and we're working on one right now...
 
Yeah, the two prostitutes laying in bed when Edward leaves the room, in the announcement trailer :)
 
Also, I think it's safe to mention that in the Watch Dogs trailer, there was a policeman with a very precise placement in one of the scenes... ;)
 
from the extracted 3d model? from the free-camera mode in PS3?

the free camera mode should be able to see everything in shower scene

Everything? So with some nifty camera work we can find out what Ellen Page had for breakfast on the morning of her body capture?
 
Fairly sure means she definitely did not do a nude scan. It's possible to get a perfect body proportion if for example they do a full body scan in the mocap suit; although the full black color wouldn't work with photogrammetry. But even a few still photographs should be enough. Then there are plenty of sources for skin textures and such to build a 3D model on top of that.

i remember Kewin Bacon telling in an interview about the movie hollow man, that they had scanned his complete nude body and he was a bit fraid of what some people with bad intentions could do with the data.

TLOU looked decent @ 30fps. But the shadow quality/filtering turned to absolute crap when 60fps was enabled.
