The rubbishness of PSN - whose fault is it?

Ok, so you shouldn't be having any NAT issues. You could try loging into that modem and assigning your PS4 to the DMZ. It will basically bypass the firewall for your PS4. You'll have to give your PS4 a static ip address. If everything works well, then you know it's probably some issue with the firewall, port forwarding, UPnP. If you put the PS4 on the DMZ and you still have issues, I'd say there's a good chance it's a line issue or an isp issue.

Also, upgrade the firmware if you haven't. Good practice for security anyway.
 
The other thing you can do is run the diagnostic tests on the modem. Seems to have a DSL synchronization test. There is a help section for a PASS/FAIL result. Don't know what it shows you. The modem should be able to report your SNR margin somewhere. You can probably check against your isp to see what the margin should be. If the margin is too low you may get intermittent issues. Not sure if that could lead to packet loss. Gaming tends to be sensitive to timeouts because you get disconnects, where other download/streaming services may have all kinds of buffers and retries that would hide the issue from you. Been a while since I've played with dsl.
 
Yes (it was set to that anyway). I've applied all the setting tweaks I've found on the internet. Again, this appeared to have worked with SWBF.
 
I've had a few issues with voice chat and connections dropping out on PSN... but I just have shoddy internet in general (HomeTelecom... grrrr).

My router is also a POS, as sometimes it just randomly decides that it doesn't like a device on the LAN and then perpetually fails to assign an IP address to that specific client, tanking the entire network in the process. I have to essentially test every device on the network one-by-one to identify which device it is and then disconnect it. It's a real pain in the backside.

Meant to follow this up. Played a few days back from 8:30 to 9:30 or something, problem free. This was shortly after the PS4 FW update.
 
I also decided to install tomato by shibby on my netgear r7000 and it seems to run great. Much better experience than when I tried the kong build of dd-wrt a while back.
