Why no audio/video sync?

Many video files exhibit a loss of sync betwenn audio and video when fast forwarding on my PC. What could be the reason for that?

What kind of files? What format? (.mpg, .avi, etc) Also, what media player are you using?

Always avi. Mostly encoded in xvid or divx. I'm using either WMP or vlc player.
Also lately I have noticed that when playing a video file in the background whilst scrolling a webpage quite fast, the sound would start to sound distorted and would only be back to normal when reopening the file.

What video card are you using and what drivers?

Sorry I forgot to mention that:
X800XT PE with Catalyst 6.1 drivers
WinXP with SP2 and all security updates and Net 2.0
Abit AV8
2x1024MB PC3200 memory

I have an Audigy 2ZS with all the latest drivers via the webupdate.
I figure the scrolling/sound problem could be the result of a poorly installed codec?
I have the K-Lite Mega Codec Pack with only the codecs I need:
Xvid, Divx, Real Alternate, Mpeg2 and AC3, thats about it.
