I think I recall SimonF stating in another forum that SEGA had T&L in quite older arcade machines (if my memory serves me well they used a number of DSP's to do the processing); no idea which model and when though.
Hey, Pyramid3D had programmable T&L (and programmable pixel ops). Probably the first "PC gaming" product to have it, although we all know the Bitboys saga... But proto P3D boards did exist, so there!
