Weird HTPC video playback issue with nVidia card

I'm having this really, really weird video playback system from my computer. It's running as a HTPC connected to a Sony 40V3000 LCD.

If I set the refresh rate to 1920x1080@24Hz and then play a 24p movie, it runs smooth for 3-4 minutes, then jerks like crazy for 15-30 seconds (looks like it's only showing every second frame).
It's not a CPU-related problem, CPU does not spike during playback, and if I check the statistics in KMPlayer it does not seem to drop frames, so the video player is sending frames at the correct rate it seems like, but they don't display correctly.

I thought it could be some kind of 24p issue, but I get a similar thing when watching DVDs, but at a different rate.
I set the output res to 1920x1080@50Hz (or 1280x720@50Hz, same result) and then play a PAL DVD movie.
Runs smooth as butter for 20-25 minutes, then jerks like crazy for 1-2 minutes, then smooth for another 20-25. It's driving me completely crazy, I can't figure out what could be causing this!
