How to capture shaders?

I've seen numerous posts here that contain shader code that was captured from an application, and I'd like to know how it's done. I'm assuming it's something with the DX9 SDK, but I really have no idea.

what if the shader is writen in HLSL? Does 3DAnalyzer still extract the asm?
