adaptive tessellation mesh swimming

Can anybody please recommend me a source of info with a solid math explanation as to why mesh swimming appears when using HW tess and how to counter it ?

I am particularly interested in the triangle tessellation because I can't get my mind on to how one can prevent the crazy HW tess patterns (fractional) from not creating a swimming effect. The way I see it when there is a jump from N -> N+2 the generated vertices will create swimming.
